West County Community Services (WCCS) has developed Transitions to Housing, a peer-developed and operated program supporting homeless or recently housed peers in their transition to permanent supportive housing (PSH). Transitions to Housing was created by our peer behavioral health workers in Monte Rio, Santa Rosa and Petaluma, to more effectively support homeless individuals with mental illness and/or addiction to reach and succeed in permanent housing.
