Job summary 1.0 FTE with benefits. $19-21 DOE

Under the general direction of the Director of Housing and Homeless Services, this position will provide intensive individual case management and crisis intervention support services to clients living in permanent supportive housing and case management and supportive services to clients currently on site. This position will also be the primary contact person for all housing issues and will work as part of a team to direct and control building operations, property management, and personnel assigned to the property.

Duties & Responsibilities
Provide effective case management for clients, including the following functions: Assess the client’s status & needs; develop a case plan; facilitate communication & coordination between service/support providers; educate the client about available resources; empower the client to problem solve in order to achieve outcomes; establish productive relationships with each and every client with no bias or prejudice towards race, religion, age, sex, disability, or sexual preference

Maintain thorough, accurate records of case management activities as required by grant contracts

Coordination and implementation of activities and programs on-site for all residents including children

Maintain HMIS or other databases necessary for reports to funders

Communicate regularly with the Housing Services Director via email, phone, incident reports, and briefings

Represent the agency in the community and work with all partner organizations to achieve the goals of clients.

Attend all meetings relevant to client care and program goals

Accountable to achieving the program’s desired outcomes with the program team.

Collaborate with the property management company and on-site manager to assist clients in understanding their rights and responsibilities.

Provide consultation in the management of differences between clients and property management

Notify property management of any current or potential behavior that would necessitate a Notices of Termination of Tenancy

Perform other related duties at the direction of the Director of Housing and Homeless Services

Qualifications
Bachelor’s degree in Social Sciences, or related field. Master’s degree is preferred.
Two or more years’ experience working with clients who have a serious mental illness and/or chronic homelessness.

Ability to relate well with others, to work on many projects simultaneously, be empathetic, firm and supportive with clients, and to work independently. Ability to work collaboratively with others serving clients with a co-occurring disorder. Knowledge of or ability to learn, clinical/Medi-Cal documentation, to work within a client-driven philosophy, and local community care system.

Knowledge and sensitivity to the needs and issues facing individuals with a history of homelessness and instability in housing. Must have good listening skills and enjoy working with people.

Ability to solve practical problems and deal with a variety of issues

Sensitivity to needs of the individual with the capacity to easily identify individual strength and build case plans from those characteristics.

Able to build relationships with local communities, service agencies and businesses in providing support and resources to formerly homeless individuals.

Ability to maintain case files, report services provided and provide monthly statistics

Enjoy working as a team member including staff and volunteers

Have a valid CA driver’s license with a good driving record and proof of insurance

Bilingual in Spanish highly desired
